What paperwork should I bring to my first appointment?

 
We have provided copies of our patient forms here. To save time when you arrive, we invite you to download, print, fill out and bring the completed forms with you to your appointment. You are also welcome to arrive 10-15 minutes early to your appointment to fill out your forms in-office if you would prefer. Please also bring a copy of your ID and insurance card to your appointment as well if you would like us to file your claims for you.

What insurances do you accept?

 
Katy Trail Dental is a non-contracted practice, which means that we are not in-network with any specific dental insurance providers. We have chosen to be non-contracted so that we can provide you with the highest quality of care and address your personal dental needs. As a courtesy service to you, we will file your dental insurance claims for your direct reimbursement. We do ask that you provide us with your insurance information so that we can research your coverage on your behalf prior to your appointment, and that you notify us if you have a change in your insurance. We will provide you with our best estimate of your treatment costs prior to providing your care.

What does it mean to be non-contracted?

 
To be non-contracted or unrestricted refers to when a patient seeks care outside the network of healthcare providers advertised, or “contracted,” by that patient’s insurance company. Most insurance plans will allow you to see any dentist of your choosing — you will simply not see us on your insurance company’s website as we do not have that advertising service in our agreement with your insurance company. In many cases, there is no difference between the patient’s cost responsibility with an in-network dentist. PPO insurance companies will allow you to see the dentist of your choosing. Our team will research your coverage prior to your appointment to make sure we can work with your insurance provider.

What if there is a price difference between you and another “in-network” dentist?

 
Our dentists and team are excited to become your dental care provider, and we never want finances to prevent you from receiving quality care. We will work with you regarding cost differences being unrestricted and will send all claims to your insurance on your behalf, as well as any written appeals needed to fund your dental treatment. This is our standard new patient policy.

How much will I have to pay for dental work?

 
Your treatment cost depends on a number of factors, including the type of treatment you receive and the severity of the problem being corrected. After your initial exam and consultation we will provide you with an estimate of what your insurance will cover. Payment is due in in full at the start of treatment. Insurance will reimburse you directly for your treatment costs.

BOTOX®? Yes, please!

 
We provide BOTOX to alleviate symptoms of bruxism, or teeth grinding, as well as to relieve orofacial pain and improve your overall appearance. Our dentists and team will perform a routine head and neck exam as well as confirm the evidence of bruxism to determine if you are a candidate for BOTOX treatment. Please be aware that we do not file medical insurance for this procedure.

Where can I park?

 
There are several convenient parking options available when you visit our office, including:
  • 2-hour parking is available on the east side of the building.
  • If your appointment ends before 3:00 p.m. you can park in the lot at Javier’s Gourmet Mexicano restaurant.
  • There is a separate lot on the southwest corner of Monticello Ave. and Cole Ave. in front of the building.

Is it safe to visit the dentist during pregnancy, and what dental issues are common?

 
Yes, it is safe and recommended to maintain regular dental check-ups or more frequent dental visits during pregnancy. Pregnant women may experience gingivitis due to hormonal changes, tooth decay, and erosion from morning sickness.
